• Conference registration will be confirmed in writing and more information on travel, hotel and activities will
be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will
be assessed. No refunds will be made if cancellation is postmarked after October 14, 2009, however,
tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the
event of conference cancellation by Symposia Medicus, full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest
you consult your own financial authority for more information.

CONFERENCE LODGING INFORMATION GROUP RATES have been arranged at the beautiful Hilton Marco Island Beach Resort & Spa. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

Please note that our block of rooms is limited and are on a first come, first serve basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.
GROUP RATES:
Garden View: $165.00/night (Single/Double Occupancy)
Gulf View: $175.00/night (Single/Double Occupancy)
Deluxe Gulf View: $185.00/night (Single/Double Occupancy)
Rooms vary only by view and are all considered junior suites. They feature private furnished balconies overlooking either the gardens or the Gulf of Mexico. Amenities in each room include: sleeper sofa, room safe, Hi-Speed Internet access (for a fee), refrigerator, coffee/tea service, LCD television with HBO, robes, bathrooms with double vanity, telephone, and hair dryer.
• Room rates are per night and do not include 10% room tax, meals, nor gratuities.
• Room rates do include unlimited fitness center admission, above stated amenities, and complimentary
newspapers (Monday-Friday).
• Maximum number of persons per room is four adults, or two adults & two children.
• Children 18 years of age and under are free in room with adults using existing bedding.
• Third & Fourth adult, 19 years and older, is an additional $25 each per night plus taxes.
• Check-in time is 4:00pm and check-out time is 11:00am.
• Please notify the hotel at check-in if there are any changes to your scheduled departure as an early
departure fee of $50 is applicable to any guest who checks out of the hotel before their stated
departure date.
• A 30 minute break has been allotted in our conference schedule on the last morning of the conference for
those who need to check out.
• Two nights' deposit must accompany the lodging reservation form. The balance of lodging payment,
as well as any incidental charges, is due to the hotel upon check out. The hotel does not accept
personal checks, but major credit cards are accepted.
• Per our hotel group contract, hotel reservation cancellations must be made in writing to Symposia
Medicus and a full refund of deposit will be made if cancellation is postmarked on or before
September 16, 2009. Between September 17, 2009, and October 14, 2009, one night's deposit is
forfeited. If cancellation is postmarked after October 14, 2009, no hotel refunds can be made.
AIR TRAVEL INFORMATION he Hilton Marco Island Beach Resort & Spa is located 30 minutes from Naples Airport, and 45 minutes from Southwest Florida International Airport, just south of Ft. Meyers, and is served by most major airlines. Miami and Ft. Lauderdale International airports are
90 minutes away.
