CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. We recommend you register early.

• Tuition:

If postmarked by August 5, 2009:
Physicians: $610.00
Nurses, Residents & PAs: $510.00

If postmarked after August 5, 2009:
Physicians: $660.00
Nurses, Residents & PAs: $560.00

• Tuition includes continuing education certification, extensive conference syllabus, group reception, and refreshments.

• Tuition may be paid by check, money order, Visa, MasterCard or Discover.

• Register on-line at www.symposiamedicus.org or by using the form in this brochure.

• Conference registration will be confirmed in writing and more information on travel, hotel and activities will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after September 9, 2009, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION

GROUP RATES have been arranged at the beautiful Presidente Inter-Continental Resort. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

You will receive these group rates by:

Please note that our block of rooms is limited and are on a first come, first serve basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.

GROUP RATES:

(Resort has 3 levels of rooms)

Garden View: $220/night (Single/Double Occupancy)
Rooms have a view of the desert gardens, and are mostly on the 2nd & 3rd levels of the hotel

Ocean View: $240/night (Single/Double Occupancy)
Rooms have a view overlooking the Sea of Cortez, and are mostly on the 2nd & 3rd levels of the hotel

Ocean View w/Balcony: $250/night (Single/Double Occupancy)
Rooms overlook the Sea of Cortez, have a private balcony with sitting area, and are mostly on the 3rd level of the hotel

Ocean Front Terrace: $290/night (Single/Double Occupancy)
Rooms are located on the 1st level of the hotel with private terrace/hammock, sitting area, and direct access to the beach

NOTE: The room rates shown above include food and beverages (except some luxury items) consumed on hotel property, taxes, service charges, hotel theme parties, use of tennis court, kid's club and exercise room.

• Maximum room occupancy is three adults OR two adults (13 years and older) and two children (12 years and younger).

• Children 5 and under are free with parents (Max 2 per room).

• Children 6-12 years are an additional $35 each per night (Max 2 per room).

• Third adult, 13 years and older, is an additional $45 per night.

• Cribs and rollaway beds are complimentary (subject to availability).

• Check in time is 3:00pm and check out time is 1:00pm.

• A 30-minute break has been allotted in our conference schedule on Saturday for those needing to check out.

• Two nights' deposit must accompany the lodging reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check out. The hotel does not accept personal checks, but major credit cards are accepted.

• Per our hotel group contract, hotel reservation cancellations must be made in writing to Symposia Medicus and a full refund of deposit will be made if cancellation is postmarked on or before August 12, 2009. Between August 13, 2009, and September 9, 2009, one night's deposit is forfeited. If cancellation is postmarked after September 9, 2009, no hotel refunds can be made.

AIR TRAVEL INFORMATION

The Presidente Inter-Continental Resort is approximately 7 miles (15 minutes) from the San Jose del Cabo airport.

Symposia Medicus has arranged group discounts with Alaska, American, and United Airlines for this conference. Reservations can be made either through your travel agency or through the airlines' Convention Desks at the numbers listed here.
