CONFERENCE REGISTRATION INFORMATION

Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

Tuition:

If postmarked by July 16, 2009:
Physicians: $610.00
Residents, Nurses & PAs: $510.00

If postmarked after July 16, 2009:
Physicians: $660.00
Residents, Nurses & PAs: $560.00

Tuition includes continuing education certification, continental breakfast, refreshments, and group reception.

Tuition may be paid by check, money order, Visa, MasterCard or Discover.

Conference registration will be confirmed in writing and more information on travel, hotel, and activities will be sent.

Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after August 20, 2009, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

This conference meets the requirements for CE deductibility under the Tax Reform Act. Please consult your financial authority for specific interpretation.

CONFERENCE LODGING INFORMATION

Group rates have been arranged at the Crowne Plaza Resort. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

Please note that our block of rooms is limited and are on a first come, first served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.

GROUP RATES:

Standard (Street View): $159/night

Courtyard View (Garden View): $179/night

Horizon View (Partial Ocean or Pool View): $209/night

All guest rooms feature wired High Speed Internet Access, a private balcony, refreshment center, coffee maker, hair dryer, iron & ironing board, in-room safe, voice-mail system, and data ports on two phone lines.

Rates are per room, per night, and do not include state and local taxes (currently 10% - subject to change), meals nor gratuities.

Maximum occupancy per room is 5.

Rollaways are currently available for $25.00 per night. Cribs are complimentary. (Subject to change)

Check in time is 4:00pm and check out time is Noon.

A 30-minute break has been allotted in our conference schedule on the last day for those needing to check out.

Self parking is free and valet parking is $10/day.

Two nights' room deposit must accompany the lodging reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check out.

Per our hotel contract, hotel cancellations must be made in writing to Symposia Medicus and you will receive a full refund if postmarked by July 23, 2009. Between July 24, 2009, and August 20, 2009, one night's deposit is forfeited. After August 20, 2009, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIR TRAVEL INFORMATION

We recommend you fly into Savannah (about a 48 mile drive to Hilton Head) as flights in/out of Hilton Head (about 7.5 miles from the hotel) are usually more expensive.

Symposia Medicus has arranged group discounts with American, Northwest and United Airlines for this conference. Reservations can be made either through each individual airlines' Convention Desk at the numbers listed below or through your travel agent.
