• Conference registration will be confirmed in writing and more information on travel, hotel and group tours
will be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will
be assessed. No refunds will be made if cancellation is postmarked after August 5, 2009, however,
tuition less 10% processing fee, may be applied to a future Symposia Medicus conference. In the
event of conference cancellation by Symposia Medicus, full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you
consult with your financial authority for more information.

CONFERENCE LODGING INFORMATION GROUP RATES have been arranged at the Vail Cascade Resort & Spa. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

Vail Cascade Resort & Spa is a full-service resort with a gourmet restaurant, heated outdoor pools, hot tubs, steam room, sauna, and the world-renowned Aria spa.
Main Lodge Room: $169/night (Single/Double) Deluxe Room: $199/night (Single/Double) Cascade Courtyard Suite: $289/night (Single/Double) Cascade Mountain View Suite: $319/night (Single/Double) ROOM AMENITIES: • Rates are per room, per night, and do not include tax (currently 9.8%) and daily resort fee of
6% (non-taxable) per room, per night, meals nor gratuities (all subject to change).
• Check in time is 4:00pm and check out time is 12:00 Noon. For a late check out inquire with the front desk
on day of departure.
• Two nights' room deposit must accompany the lodging reservation form. The balance of lodging payment,
as well as any incidental charges is due to the hotel upon check out.
• All changes in stay must be made with Symposia Medicus 30 days prior to arrival. No shows, early departures
and/or late arrivals will be charged the original contracted nightly rate for the entire reserved stay.
• Per our hotel contract, hotel cancellations must be made in writing to Symposia Medicus and you will
receive a full refund if postmarked by July 8, 2009. Between July 9, 2009, and August 5, 2009, one
night's deposit is forfeited. After August 5, 2009, no hotel refunds can be made. All cancellations
must be made in writing and postmarked by the dates stated above.
AIR TRAVEL INFORMATION
The closest airport is Eagle County Regional Airport, which is an approximate 30 minute drive to Vail; however, lower fares can often be found by flying into Denver International Airport, which is an approximate two hour drive to Vail.
Symposia Medicus has arranged group discounts with Alaska, American, Jet Blue, Northwest, and United Airlines for this conference at both airports. Reservations can be made either through your travel agent or through the airlines' Convention Desk at the numbers
