CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition includes continuing education certification, extensive conference syllabus, continental breakfasts (Wednesday, Thursday & Saturday), refreshments, and group reception.

• Tuition may be paid by check, money order, Visa, MasterCard or Discover.

• Conference registration will be confirmed in writing and more information on travel, hotel, and tours will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after January 27, 2009, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. However, we suggest you consult your financial authority for specific interpretation.

CONFERENCE LODGING INFORMATION

Symposia Medicus has reserved lodging at group rates at the Beaver Run Resort in Breckenridge, Colorado. Lodging is available to registrants and their guests at the group rates listed below. These rates will also be available three days before and after the conference (based on availability) if you wish to extend your stay.

In order to receive our group rates, you may register on-line at www.symposiamedicus.org, use the reservation form in this brochure, or call us for reservations.

Because there is a high demand for hotel rooms in Breckenridge during ski season, you should make your lodging reservations as soon as possible.

Please Note: Our block of rooms will be on a space and rate available basis after January 2, 2009. Rooms may sell out before January 2, 2009, and room rates may increase. PLEASE RESERVE EARLY!

Beaver Run Resort is a full-service resort with complete ski in/ski out access. The resort has several restaurants, a spa, two heated pools, seven hot tubs, and a sauna and steam room.

Maximum occupancy stated is regardless of age. No exceptions are made.

Hotel Room: $215/night
Maximum occupancy: 4
Two queen-size beds, full bath, and sitting area. No kitchen, fireplace or balcony.

Deluxe Studio: $235/night
Maximum occupancy: 4
One queen-size bed with sofa bed, small living room area, mini-kitchen, one bathroom and spa tub. No fireplace.

Colorado Suite: $285/night
Maximum occupancy: 4
Slightly larger than studio and has everything "Deluxe Studio" has, plus spa tub, fireplace and private balcony.

One Bedroom Suite $290/night
Maximum occupancy: 4
Separate bedroom with queen bed and queen sofa-bed in living room. One bathroom, full kitchen, fireplace, and private balcony.

Two Bedroom Suite $425/night
Maximum occupancy: 6
Three queen beds and sofa bed, full kitchen, fireplace, and private balcony.

• Rates are per room, per night, and DO NOT include sales tax of 10.68%, resort lodging fee of 3%, meals, nor gratuities. All taxes/fees are subject to change without notice.

• Two night's room deposit must accompany your registration in order to reserve a room. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check-out.

• Early departures and/or late arrivals will be charged the nightly rate for number of nights dropped. No-shows will be charged at the nightly rate for entire reserved stay.

• Conference participants and their guests can receive these group rates by registering on-line on our website at www.symposiamedicus.org, using the form in this brochure, or by calling Symposia Medicus for reservations at (800) 327-3161 or (925) 969-1789, with Visa, MasterCard, or Discover Card for deposit.

• Per our hotel group contract, hotel cancellations must be made in writing to Symposia Medicus and a full refund will be made if cancellation is postmarked on or before December 30, 2008. Between December 31, 2008, and January 27, 2009, one night's deposit is forfeited. If cancellation is postmarked after January 27, 2009, no reservation deposit refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION

Breckenridge is approximately 98 miles from Denver, Colorado. You will fly into the Denver International Airport. Information on ground transportation to Breckenridge will be sent.

Symposia Medicus has arranged group discounts with Alaska, American, Northwest and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the numbers listed here or through your travel agent.
