• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing
fee will be assessed. No refunds will be made if cancellation is postmarked after
January 20, 2009, however, tuition, less 10% processing fee, may be applied to a future
Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus,
full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest
you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION • Symposia Medicus has reserved lodging at group rates at the Beaver Run Resort in Breckenridge,
Colorado. These rates will also be available three days before and three days after the
conference, based on availability, if you wish to extend your stay.
• You will receive these special rates by reserving on-line on our website at www.symposiamedicus.org
• If paying by check, make payable to Symposia Medicus and send with registration form to

• Because there is a high demand for hotel rooms in Breckenridge during ski season, you should make
your lodging reservations as soon as possible.
• Please note that our block of rooms is limited and are on a first come, first served basis.
PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.
• Beaver Run Resort is a full-service resort with complete ski in/ski out access. The resort has several
restaurants, a spa, two heated pools, seven hot tubs, a sauna, and steam room.
• Maximum occupancy stated is regardless of age. No exceptions are made.

• Rates are per room, per night, and DO NOT include sales tax of 10.68%, resort lodging fee of 3%,
meals, nor gratuities. All taxes/fees are subject to change without notice.
• Two nights' room deposit must accompany lodging reservation form in this brochure or paid with
credit card if registering by telephone.
• Early departures and/or late arrivals will be charged the nightly rate for number of nights
dropped. No-shows will be charged at the nightly rate for entire reserved stay.
• Conference participants and their guests will receive these group rates by registering on-line on
our website at www.symposiamedicus.org, using the form in this brochure, or by calling Symposia
Medicus for reservations at (800) 327-3161 or (925) 969-1789, with Visa, MasterCard, or Discover
Card for deposit.
• Per our hotel group contract, hotel cancellations must be made in writing to Symposia Medicus and
a full refund will be made if cancellation is postmarked on or before December 23, 2008.
Between December 24, 2008, and January 20, 2009, one night's deposit is forfeited. If
cancellation is postmarked after January 20, 2009, no reservation deposit refunds can be
made. All cancellations must be made in writing and postmarked by the dates stated above.
AIRLINE TRAVEL INFORMATION Breckenridge is approximately 98 miles from Denver, Colorado. You will fly into the Denver International Airport. Information on ground
transportation to Breckenridge will be sent.
Symposia Medicus has arranged group discounts with Alaska, American, Jet Blue, Northwest and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the
