• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition includes continuing education certification, extensive conference syllabus, group reception
and refreshments.
• Tuition may be paid by check, money order, Visa, MasterCard or Discover Card.

paying by check, make payable and send with registration form to Symposia Medicus,

• Conference registration will be confirmed in writing and more information on travel and hotel
will be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition
processing fee will be assessed. No refunds will be made if cancellation is postmarked
after January 14, 2009, however, tuition, less 10% processing fee, may be applied to a
future Symposia Medicus conference. In the event of conference cancellation by Symposia
Medicus, full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act.

CONFERENCE LODGING INFORMATION Our block of rooms will be on a space and rate available basis after December 18, 2008. Rooms may sell out before December 18, 2008, and room rates may increase. PLEASE RESERVE EARLY!

Pool View: $295/night Overlooks the Inviting 26,000 sq. ft. Pool
Partial Ocean View: $333/night Best of Both Worlds: Garden & Partial Ocean View
Ocean View: $367/night Looks Out on the White Sand Beach & Ocean
All rooms are 425 sq. ft. and include a balcony, in-room safe,
on-demand movies, coffee maker, mini-refrigerator,
hair dryer, make up mirror, and iron/ironing board.
• Room rates do not include state & local taxes of 11.416% (subject to change), nor bellman
gratuities of $6.25/person (round trip) or optional maid gratuities of $2/person.
• Maximum room occupancy is four (4) persons.
• Children 12 years and under are FREE (includes full buffet breakfast & airport shuttle).
• Children 13 to 18 years old are $17 each per night (includes taxes, gratuities, full buffet breakfast
& airport shuttle).
• Extra adults in room (19 & over) are an additional $65 each per night (includes taxes, gratuities,
full buffet breakfast & airport shuttle).
• Check in time is 4:00pm and check out time is Noon.
• A 30-minute break has been allotted in our conference schedule on the Saturday for those
needing to check out.
• Two nights' deposit must accompany the reservation form. The balance of lodging
payment, as well as any incidental charges, is due to the hotel upon checkout. The hotel
accepts personal checks and most major credit cards.
• Conference participants and their guests may receive these group rates by registering on-line at
our website www.symposiamedicus.org, by using the form in this brochure, or by calling
Symposia Medicus for reservations at (800) 327-3161 or (925) 969-1789 with Visa, MasterCard,
or Discover card for deposit.
• Per our hotel group contract, hotel cancellations must be made in writing to Symposia
Medicus and you will receive a full refund if postmarked by December 17, 2008. Between
December 18, 2008 and January 14, 2009, one night's deposit is forfeited. After
January 14, 2009, no hotel refunds can be made. All cancellations must be made in
writing and postmarked by the dates stated above.
AIRLINE TRAVEL INFORMATION You will fly into the Lihue Airport on the island of Kauai.
Symposia Medicus has arranged discounts with American, Northwest, United and Hawaiian Airlines for this conference. These airlines may also offer zone fares or area pricing (fares with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the numbers listed here or through your travel agent.
