CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition:

If postmarked on/before December 3, 2008:
Physicians: $580.00
Residents, PAs & Nurses: $480.00

If postmarked after December 3, 2008:
Physicians: $630.00
Residents, PAs & Nurses: $530.00

• Tuition includes continuing education certification, extensive conference syllabus, group reception, and refreshments.

• Tuition may be paid by check, money order, Visa, MasterCard or Discover.

• Conference registration will be confirmed in writing and more information on travel, hotel, and activities will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after January 7, 2009; however, tuition (less 10% processing fee) will be applied toward a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition and hotel deposit refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. However, we suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION

GROUP RATES have been arranged at the beautiful Kauai Marriott Resort. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

Our block of rooms will be on a space and rate available basis after December 11, 2008. Rooms may sell out before December 11, 2008, and room rates may increase. PLEASE RESERVE EARLY!

All rooms are 425 sq. ft. and include a balcony, in-room safe, on-demand movies, coffee maker, mini-refrigerator, hair dryer, make up mirror, and iron/ironing board.

• Room rates do not include state & local taxes of 11.416% (subject to change), nor bellman gratuities of $6.25/person (round trip) or optional maid gratuities of $2/day.

• Maximum room occupancy is four (4) persons.

• Children 12 years and under are FREE (includes full buffet breakfast & airport shuttle).

• Children 13 to 18 years old are $17 each per night (includes taxes, gratuities, full buffet breakfast & airport shuttle).

• Extra adults in room (19 & over) are an additional $65 each per night (includes taxes, gratuities, full buffet breakfast & airport shuttle).

• Check-in time is 4:00pm and check-out time is Noon.

• A 30-minute break has been allotted in our conference schedule on Saturday for those needing to check out.

• Two nights' deposit must accompany the reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon checkout. The hotel accepts personal checks and most major credit cards.

• Conference participants and their guests may receive these group rates by registering on-line at our website www.symposiamedicus.org, by using the form in this brochure, or by calling Symposia Medicus for reservations at (800) 327-3161 or (925) 969-1789 with Visa, MasterCard, or Discover Card for deposit.

• Per our hotel group contract, hotel cancellations must be made in writing to Symposia Medicus and you will receive a full refund of hotel deposit if postmarked by December 10, 2008. Between December 11, 2008, and January 7, 2009, one night's deposit is forfeited. After January 7, 2009, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION

You will fly into the Lihue Airport on the island of Kauai.

These airlines may also offer zone fares or area pricing (fares with no Saturday night stay requirements).
