CONFERENCE REGISTRATION INFORMATION

If postmarked after October 10, 2008:
Physicians: $530.00
Residents, PA's & Nurses: $445.00

• Tuition includes continuing education certification, extensive conference syllabus, continental breakfasts and refreshments.

• Tuition may be paid by check (payable to Symposia Medicus), money order, Visa, MasterCard or Discover Card.

• Conference registration will be confirmed in writing and more information on the conference, travel and hotel will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after November 14, 2008, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your financial authority for more information.

HOTEL RESERVATION INFORMATION

(Reserve Early - Hotel Rooms at our Group Rates Sell Out Fast)

Symposia Medicus has reserved a block of rooms with special group rates at the Hilton New York Hotel & Towers located in the heart of Manhattan at 1335 Avenue of the Americas (on 6th Avenue, between 53rd & 54th Streets).

Our block of rooms will be on a space and rate available basis after October 17, 2008 or WHEN OUR ROOM BLOCK SELLS OUT, whichever occurs first. Our group rates WILL NOT be available after cut-off date or rooms in block have been sold. Please reserve early!!

GROUP RATES (Single/Double Occupancy):

Run of House - $299/night
All guest rooms feature marble baths, in-room thermostat control, 27 inch television, on-demand entertainment including video games, refreshment center, spacious work area, coffeemaker, and two telephones with voicemail and dataport.

Executive Level - $345/night*
Executive accommodations (on higher floors of the hotel) include the same features as the standard rooms and have in addition upgraded decor, bottled water, robes and access to 44th floor Executive Lounge featuring continental breakfast daily, hors d'oeuvres in the afternoon and evening as well as continuous coffee and tea service. (Executive Level rooms offer separate check-in and concierge service.)

NOTE:

• Rates do not include state & city tax (currently 13.63%) per room per night, $2.00 per room per night secondary tax, and $1.50 per room per night tertiary tax. All taxes are subject to change without prior notice.

• Third & fourth persons in a room are $30.00 each per night extra.

• Maximum persons in room are four.

• Children 18 and under are complimentary, using existing bedding, when staying with adults.

• Check in time is 3:00pm and Check out time is Noon.

• One night's deposit (room rate only) must accompany your reservation. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon checkout. The hotel accepts all major credit cards.

• For any changes to your reservation, please contact Symposia Medicus at (800) 327-3161 or (925) 969-1789.

• Hotel cancellations must be made in writing to Symposia Medicus and you will receive a full refund if cancellation is postmarked by November 14, 2008. After November 14, 2008, no hotel refunds can be made. All cancellations must be made in writing, sent to Symposia Medicus and postmarked by the dates stated above.

AIR TRAVEL INFORMATION

Symposia Medicus has arranged for discount fares with Alaska, American, Northwest, and United Airlines for this conference. These airlines may also offer zone fares (flat rates with no Saturday night stay requirements). Reservations can be made either through your own travel agency or through the airlines Convention Desk at the numbers listed below.

In order to receive the discounted rate, you or your travel agent must refer to the Symposia Medicus Group ID number listed below.

CAR RENTAL DISCOUNT

Symposia Medicus has made arrangements with AVIS Car Rentals for conference discounts. Call the AVIS Convention Desk at 1-800-331-1600 for reservations. Please refer to the AVIS meeting code #B766625 and the location when making your reservation to get the discount.

HOLIDAY ENTERTAINMENT AND THEATER ASSISTANCE

During this busy holiday time in New York City, it is difficult to get theater tickets, so we have purchased a block of theater tickets for a variety of popular performances during your stay (see to the right). We have a limited number of tickets for each performance and anticipate they will all sell out, so we recommend you call as soon as possible. The seats are good (most are in the Orchestra or Mezzanine). To order tickets, call Symposia Medicus with Visa, MasterCard, or Discover at (925) 969-1789. No ticket refunds or exchanges can be made.
