Conference registration will be confirmed in writing and more information on the conference, travel and hotel will be sent.
Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed.
No refunds will be made if cancellation is postmarked after November 14, 2008, however, tuition, less 10% processing
fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia
Medicus, full tuition refunds will be made.
This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your financial
authority for more information.

HOTEL RESERVATION INFORMATION Symposia Medicus has reserved a block of rooms with special group rates at the Hilton New York Hotel & Towers located in the heart of Manhattan at 1335 Avenue of the Americas (on 6th Avenue, between 53rd & 54th Streets).
You can receive these special rates by reserving online at www.symposiamedicus.org.

Because hotel accommodations are in high demand in New York City during this holiday season, we
recommend you make your hotel reservations as soon as possible.
Our block of rooms will be on a space and rate available basis after October 17, 2008 or when our room block sells out, whichever occurs first. Our group rates WILL NOT be available after the cut-off date or rooms in our block have been sold. Please reserve early!!

NOTE:

Rates do not include city & state taxes (currently 13.63%) per room per night, a $2.00 per room per night secondary tax,
and a $1.50 per room per night tertiary tax. All taxes are subject to change without prior notice.
Third and fourth guests in room are $20.00/person/night.
Maximum guest room capacity is four.
Children 18 & younger stay free in room with parents.
Check-in time is 3:00 p.m. Ð Check-out time is Noon.
One night's deposit (room rate only) must accompany your reservation. The balance of lodging payment, as well as any
incidental charges, is due to the hotel upon checkout. The hotel accepts all major credit cards.
Per our hotel contract, cancellations must be made in writing to Symposia Medicus and you will receive a full refund
of your deposit if cancellation is postmarked by November 14, 2008. After November 14, 2008, no hotel deposit
refunds can be made. All cancellations must be made in writing, sent to Symposia Medicus and postmarked by the
dates stated above.
AIR TRAVEL INFORMATION Symposia Medicus has arranged for discount fares with Alaska, American, Northwest, and United Airlines for this conference. These airlines may also offer zone fares (flat rates with no Saturday night stay requirements). Reservations can be made either through your travel agent or through the airlines Convention Desk at the numbers listed.
In order to receive the discounted rate, you or your travel agent must refer to the Symposia Medicus Group ID
number listed below.

HOLIDAY ENTERTAINMENT AND THEATER ASSISTANCE
During this busy holiday time in New York City, it is difficult to get theater tickets, so we have
purchased a block of theater tickets for a variety of popular performances during your stay (see right). We have a limited number of tickets for each performance and anticipate they will all sell out, so we recommend you call as soon as possible. The ticket prices are 25% over what you would pay directly to theater, but the seats are good (most are in the Orchestra or Mezzanine). To order
tickets, call Symposia Medicus with Visa, MasterCard, or Discover at (925) 969-1789.
No ticket refunds or exchanges can be made.
