CONFERENCE REGISTRATION INFORMATION

Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

Tuition Fees:

If postmarked by September 24, 2008:
Physicians: $580.00
Residents, PA's & Nurses: $480.00

If postmarked after September 24, 2008:
Physicians: $630.00
Residents, PA's & Nurses: $530.00

Tuition includes continuing education certification, extensive conference syllabus, continental breakfast on Wed., Thurs., & Sat., refreshments, and group reception.

Tuition may be paid by check, money order, Visa, MasterCard or Discover Card. If paying by check, make payable and send with registration form to Symposia Medicus, 399 Taylor Blvd., Suite 201, Pleasant Hill, CA 94523-2200.

Telephone registration will be taken with Visa, MasterCard or Discover Card. Telephone registrations should be made Monday-Friday, 8:30am-5:00pm (Pacific) by calling (800) 327-3161 or (925) 969-1789. Registrations may also be faxed with credit card payment to (925) 969-1795.

Conference registration will be confirmed in writing and more information on travel, hotel and tours will be sent.

Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is post- marked after October 29, 2008, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION

Symposia Medicus has reserved accommodations at group rates at the Hilton Marco Island Beach Resort, Marco Island, Florida. Accommodations are available to conference participants and their guests at the rates listed below. These rates will also be available three days before and three days following the conference, based on availability. In order to qualify for these group rates, you must use the reservation form in this brochure.

Make your lodging reservations as soon as possible by using the form in this brochure. Our block of rooms will be on a space and rate available basis only after October 8, 2008. Rooms may sell our before October 8, 2008 and room rates may increase. PLEASE RESERVE EARLY!

GROUP RATES (Single or Double Occupancy):

Garden View: $159.00/night

Partial Gulf View: $169.00/night

Full Gulf View: $179.00/night

Rooms only differ by view and floor level

Room rates are per night and do not include 10% room tax, meals, nor gratuities.

Room rates include unlimited fitness center admission, in-room coffee and tea, and complimentary newspaper.

Maximum number of persons per room is four adults, or two adults & two children.

Children under 18 years of age are free in room with adults using existing bedding.

Third and fourth adult (18 and over) in room are $25.00 each per night.

A two nights' deposit must accompany your registration in order to reserve a room. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check-out.

Check-in time is 4:00pm and check-out time is 11:00am.

Self parking is $8.00 per day for hotel guests. Valet parking is available for $12.00 per day for hotel guests only.

A 30 minute break has been allotted in our conference schedule on the last morning of the conference for those who need to check out.

Please notify the hotel at check-in if there are any changes to your scheduled departure, to avoid being charged an early checkout fee of $50.00.

Conference participants and their guests may receive these group rates by registering online on our website at www.symposiamedicus.org, using the form in this brochure, or by calling Symposia Medicus for reservations at (800) 327-3161 or (925) 969-1789 with Visa, MasterCard, or Discover Card for deposit.

Per our hotel group contract, hotel cancellations must be made in writing and a full refund of hotel deposit will be made if postmarked by October1, 2008. Between October 2, 2008, and October 29, 2008, one night's deposit is forfeited. After October 29, 2008, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION

The Hilton Marco Island Beach Resort is located 30 minutes from Naples Airport, and 45 minutes from Southwest Florida International Airport, just south of Ft. Meyers, and is served by most major airlines. Miami and Ft. Lauderdale International airports are 90 minutes away.

Symposia Medicus has arranged for group discounts with American, Northwest, and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through your travel agency or through the airlines' Convention Desk at the numbers here.
