REGISTRATION INFORMATION Enrollment for this conference is limited in order to provide the best possible learning environment. We recommend you register early. Tuition Fees:

breakfast on Wednesday, Thursday, & Saturday (for conference attendees only), group reception,
and refreshments.
Tuition may be paid by check, money order, Visa, MasterCard or Discover Card.

Conference registration will be confirmed in writing and more information on travel and hotel will
be sent.
Conference cancellations must be confirmed in writing and a 10% of paid tuition processing
fee will be assessed. No refunds will be made if cancellation is postmarked after October 15,
2008, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus
conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds
will be made.
This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest
you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION You qualify for our group rates by using the reservation form in this brochure or by registering on-line on our website at www.symposiamedicus.org. Our block of rooms will be on a space and rate available basis after September 19, 2008. Rooms may sell out before September 19, 2008 and room rates may increase. PLEASE RESERVE EARLY!
GROUP RATES (Single/Double Occupancy):
Royal Tower Terrace View (Views of terrace, gardens or courtyard) Royal Tower Water View (Views of pools, harbor or ocean) Rates are per night and do not include Resort Levy of 12% per day, nor gratuities & resort fees.
Gratuities & resort fees are as follows:
One time bellman gratuity charge of $5.70 per person (age 12 and older)
Housekeeping gratuity of $4.00 per person, per day (age 12 and older)
Utility service fee (government tax for electricity) of $7.50 per person, per day
(age 12 and older)
Pool/beach attendant/lifeguard fee of $1.50 per person, per day (age 12 and older)
Additional gratuities are at your discretion.
Third person (age 12 and older) in room is $35.00 extra per night, plus resort levy, and gratuities
& resort fees as stated above.
Children 11 and under are complimentary, using existing bedding, when staying with adults.
Maximum guest room capacity is four adults (age 12 and older), or two adults and two children
under age 12.
Check-in time is 3:00pm and check-out time is 11:00am.
NOTE: Hotel check-out time is 11:00am. If you check-out after 11:00am, an automatic late
charge of $150 will be added to your final hotel bill. If late check-out is needed, please
inquire with hotel front desk on the date of departure.
A 30-minute break has been allotted in our conference schedule on Saturday for hotel check-out,
if applicable.
Two nights' deposit must accompany the reservation form. The balance of lodging payment,
as well as resort levy, gratuities & resort fees, is due to the hotel upon check-out. The hotel
accepts all major credit cards.
Conference participants and their guests can receive these group rates by using the form in this
brochure, by registering on line on our website at www.symposiamedicus.org, or by calling
Symposia Medicus for reservations at (800) 327-3161 or (925) 969-1789 with Visa, MasterCard,
or Discover Card.
Per our hotel group contract, hotel cancellations must be made in writing to Symposia
Medicus and a full refund of hotel deposit will be made if postmarked by September 17, 2008.
Between September 18, 2008, and October 15, 2008, one night's deposit is forfeited. After
October 15, 2008, no hotel refunds can be made. All cancellations must be made in writing
and postmarked by the dates stated above.
AIRLINE TRAVEL INFORMATION
You will fly into Nassau International Airport. The airport is approximately
15 miles from the Atlantis Resort on Paradise Island. You can take a taxi to the resort (fare is approximately $30), or rental cars are available. The resort has free self-parking.
Symposia Medicus has arranged group discounts with American and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the
numbers listed here or through your travel agent.
