CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition Fees:

If postmarked on/before September 10, 2008:
Physicians: $580.00
Residents, PA's & Nurses: $480.00

If postmarked after September 10, 2008:
Physicians: $630.00
Residents, PA's & Nurses: $530.00

• Tuition includes continuing education certification, extensive conference syllabus, continental breakfast on Wednesday, Thursday, and Saturday (for conference attendees only), group reception, and refreshments.

• Telephone registrations will be taken with Visa, MasterCard or Discover Card. Telephone registrations can be made Monday-Friday, 8:30am-5:00pm (Pacific Time) by calling (800) 327-3161 or (925) 969-1789. Registrations may also be faxed with credit card information to (925) 969-1795.

• Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card. If paying by check, make payable and send with registration form to Symposia Medicus, 399 Taylor Blvd., Suite 201, Pleasant Hill, CA 94523-2200.

• Conference registration will be confirmed in writing and more information on travel, hotel and group tours will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after October 15, 2008, however, tuition less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION

Symposia Medicus has reserved accommodations at the Crowne Plaza Resort. Accommodations are available to registrants and their guests at the conference group rates listed below. These rates will also be available three days before and three days after the conference, based on availability.

Make your lodging reservations as soon as possible by registering on-line at www.symposiamedicus.org, by calling Symposia Medicus for reservations at (800) 327-3161 or (925) 969-1789 with Visa, MasterCard or Discover Card, or by using the form in this brochure. Our block of rooms will be on a space and rate available basis after September 19, 2008. Rooms may sell out before September 19, 2008, and room rates may increase. PLEASE RESERVE EARLY.

• GROUP RATES (Single/Double Occupancy):

Standard (Street View): $149/night

Courtyard View (Garden View): $169/night

Horizon View (Partial Ocean or Pool View) : $199/night

• All rooms are the same, differing only by view. All guest rooms feature wired High Speed Internet Access, a private balcony, refreshment center, coffee maker, hair dryer, iron & ironing board, in-room safe, voice-mail system, and data ports on two phone lines.

• Rates are per room, per night, and do not include state and local taxes (currently 10%), meals nor gratuities.

• Children 19 and under stay free in room with parents and existing bedding.

• Rollaways are available for $25.00 per night. Cribs are complimentary.

• Check in time is 4:00pm and check out time is Noon.

• A 30-minute break has been allotted in our conference schedule on Saturday for those needing to check out.

• Two nights' room deposit must accompany the lodging reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check out.

• Hotel cancellations must be made in writing and a full refund will be made if cancellation is postmarked on or before September 17, 2008. Between September 18, 2008, and October 15, 2008, one night's deposit is forfeited. If cancellation is postmarked after October 15, 2008, no hotel refunds can be made.

AIR TRAVEL INFORMATION

We recommend you fly into Savannah (about a 48 mile drive to Hilton Head) as flights in/out of Hilton Head (about 7.5 miles from the hotel) are more expensive.

Symposia Medicus has arranged group discounts with American, Northwest and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the numbers listed here or through your travel agent.
