• Tuition includes continuing education certification, extensive conference syllabus, continental
breakfast on (for conference attendees only), group reception, and refreshments.

• Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card. If paying
by check, make payable and send with registration form to Symposia Medicus, 399 Taylor
Blvd., Suite 201, Pleasant Hill, CA 94523-2200.
• Telephone registrations will be taken with Visa, MasterCard or Discover Card. Telephone
registrations can be made Monday-Friday, 8:30 am-5:00 pm (Pacific Time) by calling
(800) 327-3161 or (925) 969-1789. Registrations may also be faxed with credit card
information to (925) 969-1795.
• Conference registration will be confirmed in writing and more information on travel, hotel and
group tours will be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition
processing fee will be assessed. No refunds will be made if cancellation is postmarked
after October 1, 2008, however, tuition less 10% processing fee, may be applied to a
future Symposia Medicus conference. In the event of conference cancellation by
Symposia Medicus, full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act.

CONFERENCE LODGING INFORMATION Make your lodging reservations as soon as possible by using the form in this brochure.
Our block of rooms will be on a space and rate available basis after October 1, 2008.
Rooms may sell out before October 1, 2008 and room rates may increase.

GROUP RATES:
Fairmont Room - $191.00 Deluxe Casita - $221.00 Signature Casita - $251.00 Rates do not include meals, the Value Added tax (V.A.T.) of 10% per room, per day, lodging tax of 2% per room, per day and resort fee of $12.00 per room, per day (resort fee includes porterage and housekeeping gratuities - Additional gratuities are at your discretion) - subject to change.
• 3rd and 4th adult (19 years old and over) is $30 per person, per day.
• Children 18 and under are complimentary, using existing bedding, when staying with adults.
• Maximum number in room is 4 people.
• Check-in time is 4:00pm and check-out time is 12:00pm.
• A 30-minute break has been allotted in our conference schedule on the last day for those of you
who need to check out of the hotel.
• Children 6-12 years get 50% off at hotel restaurants and children 5 and under eat for free
(based on children's menu).
• Two nights' deposit must accompany the reservation in order to reserve a room. The
balance of lodging payment, as well as any incidental charges, is due to the hotel upon
checkout. The hotel does not accept personal checks, but major credit cards and cash
are accepted.
• Conference participants and their guests may receive these special group rates by using the
form in this brochure, by calling Symposia Medicus for reservations at (800) 327-3161 or
(925) 969-1789 with Visa, MasterCard, or Discover Card for deposit, or by registering on-line
on our website at www.symposiamedicus.org.
• Hotel cancellations must be made in writing and you will receive a full refund if
postmarked by September 10, 2008. Between September 11, 2008 and October 8, 2008,
one night's deposit is forfeited. After October 8, 2008 no hotel refunds can be made. All
cancellations must be made in writing and postmarked by the dates stated above.
AIR TRAVEL INFORMATION You will fly into the Cancun International Airport, which is
approximately 42 miles (40 minutes) from the Fairmont Mayakoba Resort. Additional Information regarding ground
transportation options will be sent prior to the conference.
Symposia Medicus has arranged group discounts with Alaska, American, Northwest, and United Airlines for this program. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the numbers
listed here or through your travel agent.

Lodging is available to registrants and their guests at the rates listed below. These rates will also be available three days before and after the conference (based on availability) if you wish to extend your stay.
