CONFERENCE REGISTRATION INFORMATION

Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

Tuition Fees:

If postmarked by August 27, 2008:
Physicians: $580.00
Residents, PA's & Nurses: $480.00

If postmarked after August 27, 2008:
Physicians: $630.00
Residents, PA's & Nurses: $530.00

Tuition includes continuing education certification, extensive conference syllabus, group reception and refreshments.

Tuition may be paid by check, money order, Visa, MasterCard or Discover Card.

Telephone registrations can be made Monday through Friday, 8:30am-5:00pm (Pacific Time), by calling (925) 969-1789 or (800) 327-3161. Registrations can also be faxed with Visa, MasterCard or Discover Card information to (925) 969-1795.

Conference registration will be confirmed in writing and more information on travel and hotel will be sent.

Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after October 1, 2008, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION

Group rates have been arranged at the Wailea Marriott Resort. These rates will also be available three days before and three days after the conference, based on availability, if you wish to extend your stay. You qualify for our group rates by registering online on our website at www.symposiamedicus.org or by using the reservation form in this brochure.

Our block of rooms will be on a space and rate available basis after September 5, 2008. Rooms may sell out before September 5, 2008, and room rates may increase. PLEASE RESERVE EARLY!

GROUP RATES (Single/Double Occupancy) - Includes full buffet breakfast:

Terrace View: $245/night

Ocean View: $280/night

Ocean Front: $300/night

All rooms have a balcony, and either a king or two double beds. Rooms with a King bed also have a sleeper sofa.

Terrace View rooms are located throughout the resort and have views of the lushly landscaped grounds.

Ocean View rooms are located closer to the ocean with ocean views from the balcony.

Ocean Front rooms are located closest to the ocean and are only on the 2nd & 3rd floors. All rooms are equipped with a hair dryer, cable color TV, safe, mini refrigerator, iron and ironing board, and table with two chairs on the balcony.

Children 12 years and under are FREE in room with adults (includes full buffet breakfast)

Children 13-18 years old are $17/person/night (includes taxes, gratuities & full buffet breakfast)

3rd & 4th adult in room (19 and over) are $59/person/night (includes taxes, gratuities & full buffet breakfast)

Maximum number of people in room is four.

Room rates do not include current state and local taxes of 11.416% (subject to change).

Check-in time is 4:00pm; check-out time is Noon.

A 30-minute break has been allotted in our conference schedule on Saturday for those of you who need to check out.

Two nights' deposit must accompany the reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon checkout. The hotel accepts all major credit cards.

Conference participants and their guests can receive these special rates only by using the form in this brochure or by registering on our web-site at www.symposiamedicus.org or by calling Symposia Medicus for reservations with Visa, MasterCard or Discover Card information for deposit.

Hotel cancellations must be made in writing and you will receive a full refund if post- marked by September 3, 2008. Between September 3, 2008, and October 1, 2008, one night's deposit is forfeited. After October 1, 2008, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION

You will fly into the Kahului Airport, which is approximately 17 miles from the Marriott Wailea Beach Resort & Spa.

Symposia Medicus has arranged group discounts with American, Hawaiian, Northwest and United Airlines for this program. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the numbers listed here or through your travel agent.
