REGISTRATION INFORMATION
• Enrollment for this cruise conference is limited. Early
registration is advised.
• Tuition Fees:
If postmarked by August 8, 2008: If postmarked after August 8, 2008: • Tuition includes continuing education certification and an
extensive conference syllabus.
• Register on-line at www.symposiamedicus.org OR by
using the form in this brochure.
• Tuition may be paid by check, money order, Visa,
MasterCard, or Discover Card. If paying by check, make
payable and send with registration form to Symposia
Medicus, 399 Taylor Blvd., Suite 201, Pleasant Hill, CA
94523-2200.
• Telephone registrations will be taken with Visa,
MasterCard or Discover Card. Telephone registrations
should be made Monday-Friday, 8:30am-5:00pm (Pacific
Time) by calling (800) 327-3161 or (925) 969-1789.
Registrations may also be faxed with credit card number
to (925) 969-1795.
• Conference registration and cruise reservations will be
confirmed in writing and more information on the conference,
travel, and cruise ship will be sent to you.
• Conference cancellations must be confirmed in writing and a
10% of paid tuition fee will be assessed. No tuition refunds
will be made if cancellation is postmarked after September
12, 2008. However tuition, less 10% processing fee, can be
applied to a future Symposia Medicus program. In the event
of program cancellation by Symposia Medicus, full tuition
refunds will be made. Please see Cruise Cancellation
Policy section regarding cruise payment refunds.

• CRUISE CABIN RATES
Rates are PER PERSON, based on double occupancy, and
DO NOT include cruise fees of $159 per person, taxes of
$59.13 per person (subject to change), fuel surcharge of
$5 per day per person, cabin gratuities of $68.25 per person
or suite gratuities of $84 per person, airfare, ground
transportation, nor cruise cancellation insurance.

• Total due at booking is $250 per person
• Final Payment due to Symposia Medicus by Friday,
July 25, 2008.
• Single occupancy fares are available by request, and subject
to availability. The rate for single occupancy is 200% of the
fare for selected accommodations.
• Third & fourth persons in a cabin, with two full-fare
passengers, are $240 for each additional person, plus
$159 per person cruise fees, $59.13 per person taxes,
$68.25 per person gratuities or $84 per person gratuities
for suites.
• Not all cabins can accommodate triples and/or quads.
Triple and quad cabins are limited. Please call us as
soon as possible if you are interested. All cabin
categories are subject to availability.
