CONFERENCE REGISTRATION INFORMATION

Enrollment for this conference is limited in order to provide the best possible learning environment. We recommend you register early.

Tuition:

If postmarked by August 13, 2008:
Physicians: $580.00
Residents, PA's & Nurses: $480.00

If postmarked after August 13, 2008:
Physicians: $630.00
Residents, PA's & Nurses: $530.00

Tuition includes continuing education certification, extensive conference syllabus, group reception, and refreshments.

Tuition may be paid by check, money order, Visa, MasterCard or Discover Card.

Conference registration will be confirmed in writing and more information on travel and hotel will be sent.

Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after September 17, 2008, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your own financial authority for more information.

CONFERENCE LODGING INFORMATION

all-inclusive group rates have been arranged at the lovely Presidente Inter-Continental Resort in Los Cabos, Mexico. These rates will also be available three days before and three days after the conference, based on availability, if you wish to extend your stay.

You qualify for our rates by using the reservation form in this brochure or by registering online on our website at www.symposiamedicus.org. Our block of rooms will be on a space and rate available basis only after August 22, 2008. Rooms may sell out before August 22, 2008, and room rates may increase. PLEASE RESERVE EARLY!

GROUP RATES BELOW ARE "ALL-INCLUSIVE." INCLUDES ALL FOOD & BEVERAGE, TAXES & GRATUITIES

Garden View - $234/night
Rooms have view of the desert gardens, and are on the 2nd & 3rd level of the hotel.

Ocean View - $254/night
Rooms have a view overlooking the Sea of Cortes and are on the 2nd & 3rd levels of the hotel.

Ocean View w/Balcony - $274/night
Rooms overlook of the Sea of Cortes, and have a private balcony with sitting area, and are on the 3rd level of the hotel.

Ocean Front Terrace - $315/night
Rooms are located on the 1st level of hotel, with private terrace, sitting area, and direct access to the beach.

NOTE: The room rates shown above include all food and beverages consumed on hotel property, taxes, gratuities, theme parties, and use of tennis court, kids club, and exercise room.

Maximum room occupancy is four - up to three adults or two adults and two children ages 12 & under.

Children 5 & under are free with parents.

Child 6-12 years are $35 each per night.

Third adult 13 & over are $45 each per night.

Hotel check-in is 3:00pm and check-out time is 1:00pm.

A 30-minute break has been allotted in our conference schedule on the last day for those of you who need to check out.

Two nights' deposit must accompany the reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon checkout. The hotel does not accept personal checks, but major credit cards are accepted.

Conference participants and their guests may receive these special rates only by using the form in this brochure or by calling Symposia Medicus for reservations with Visa, MasterCard or Discover Card number for deposit. Call Monday-Friday, 8:30am-5:00pm (PT) at (800) 327-3161 or (925) 969-1789.

Hotel cancellations must be made in writing and you will receive a full refund if postmarked by August 20, 2008. Between August 21, 2008 and September 17, 2008 one night's deposit is forfeited. After September 17, 2008, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

SPECIAL AIRLINE DISCOUNTS

The Presidente Inter-Continental Resort is approximately 7 miles (15 minutes) from the San Jose del Cabo airport.

Symposia Medicus has arranged group discounts with Alaska, American, Northwest, and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through your own travel agency or through the airlines' Convention Desks at the numbers listed here.
