CONFERENCE REGISTRATION INFORMATION

Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

Tuition:

If postmarked on/before July 17, 2008:
Physicians: $580.00
Residents, PA's & Nurses: $480.00

If postmarked after July 17, 2008:
Physicians: $630.00
Residents, PA's & Nurses: $530.00

Tuition includes continuing education certification, extensive conference syllabus, continental breakfasts & refreshments.

Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card. If paying by check, make payable and send with registration form to Symposia Medicus, 399 Taylor Blvd., Suite 201, Pleasant Hill, CA 94523-2200.

Telephone registrations will be taken with Visa, MasterCard or Discover Card. Telephone registrations should be made Monday-Friday, 8:30am-5:00pm (Pacific Time) by calling (800) 327-3161 or (925) 969-1789. Registrations may also be faxed with credit card number to (925) 969-1795.

Conference registration will be confirmed in writing and more information on travel and hotel will be sent.

Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after August 12, 2008, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus program. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION

Symposia Medicus has reserved accommodations at group rates at The Westin Gaslamp Quarter, San Diego (formerly Westin Horton Plaza) for your stay during our conference. Accommodations are available to conference participants and their guests at the group rates listed below. These rates will also be effective three days before and three days after the conference based on availability.

NOTE: Our block of rooms will be on a space and rate available basis after August 22, 2008. Rooms may sell out before August 22, 2008, and room rates may increase. PLEASE RESERVE EARLY.

Traditional Room: $253.00 (Single/Double Occupancy)
Maximum Occupancy - 4 persons
Spacious guestroom with stylish urban design, Flat screen TV, 6ft desk, Minibar, Windows that open, Non-Smoking, Heavenly Bed and Bath, High-speed internet (for a fee), 2-line Speakerphone with dataport, and In-room safe.

Deluxe Room: $283.00 (Single/Double Occupancy)
Maximum Occupancy - 4 persons
Higher floor spacious guestroom with stylish urban design, Flat screen TV, 6ft Desk, Minibar, Complimentary Bottled Water, Non-Smoking, Heavenly Bed and Bath, Free High-speed Internet Access, 2-line Speakerphone with dataport, and In-room safe.

One night's room deposit, including tax is due at time of reservation.

Rates are per room, per night, and do not include combined state and local taxes of 12.565%, (subject to change), meals, nor gratuities.

Third and fourth persons 18 and over in room are $20.00 per person, per night. There is a maximum of 4 people per room.

Children 17 and under stay free in room with adult with existing bedding.

Check-in time is 3:00pm. Checkout time is noon. Late checkout may incur a charge.

In the event that you must check out earlier than your original departure date a one time fee of $75.00 will be incurred. Hotel will confirm check-out date at the time of check-in and fee will only be incurred if date changes after this confirmation.

Room reservation cancellations made within 72 hours of scheduled arrival will result in a forfeiture of the one night's room and taxes.

Symposia Medicus has arranged group discounts with Alaska, American, Northwest, and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements).
