REGISTRATION INFORMATION
• Enrollment for this conference is limited in order to provide the best
possible learning environment. Early registration is advised.
• Tuition:

conference syllabus, continental breakfast (Tuesday and Wednesday),

• Conference registration will be confirmed in writing and more
information on travel and hotel will be sent.
• Conference cancellations must be confirmed in writing and a
10% of paid tuition processing fee will be assessed. No refunds
will be made if cancellation is postmarked after August 18, 2008,
however, tuition, less 10% processing fee, may be applied to a
future Symposia Medicus conference. In the event of conference
cancellation by Symposia Medicus, full tuition refunds will be
made.
• This conference meets the requirements for CE deductibility under the
Tax Reform Act. We suggest you consult your financial authority for
more information.

Symposia Medicus has reserved a block of rooms at group rates at the Riviera Hotel & Casino. The Riviera Las Vegas is a
self-contained casino and resort with everything to offer its guests. The hotel, located right on the famous Las Vegas Strip, combines a resort atmosphere with the glamour and excitement of Las Vegas.
Lodging is available to conference
participants and their guests at the rates listed here. These rates will also be effective three days before and three days after the conference if you wish to extend your stay in Las Vegas (based
on room and rate availability).
Our block of rooms will be on a space and rate available basis after August 18, 2008. Rooms may sell out before August 18, 2008, and room rates may increase. PLEASE RESERVE EARLY!
GROUP RATE: $175.00 Petite Suites (Single/Double Occupancy) Note:
• Rates do not include tax (currently 9%), meals, nor gratuities.
• Third & fourth persons in room are $20 extra per person, per night.
• Maximum four guests per room.
• Rollaway beds are complimentary.
• Check in time is 4:00pm and check out time is 11:00am.
• A 30-minute break has been allotted in our conference schedule on the last day, for those of you who need to check out
of the hotel.
• Hotel cancellations must be made directly with the hotel 72 hours prior to arrival or you will be charged a cancellation
fee equal to your one night's deposit & tax.

These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each
