CONFERENCE DESCRIPTION
This exciting conference is designed to provide physicians, nurse practitioners, nurses, and physician assistants with evidenced-based information on current medical issues facing healthcare professionals. The goal of this conference is to enhance the knowledge of healthcare providers, thereby improving patient care. In addition to didactic lectures, which will present current diagnostic and management information, ample time has been allotted for participant feedback and interaction.

Conference topics have been selected based on past participant conference evaluation requests, clinical expert input, literature review, and our online needs assessment surveys of health care providers.

An outstanding faculty has been assembled for this program. Faculty members have been selected because of their expertise, their ability to communicate first-hand knowledge to others, their commitment to address issues of current concern, and their willingness to interact with participants.

ACCREDITATION
This educational activity was planned and produced in accordance with the ACCME and ANCC essentials and standards relating to continuing medical and nursing education.

Symposia Medicus is accredited by the Accreditation Council on Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

Symposia Medicus is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation (ANCC).

CONTINUING EDUCATION CREDIT

Physicians
Symposia Medicus designates this educational activity for a maximum of 15 AMA PRA Category 1 Credits™.

Application has been made to the American College of Emergency Physicians for ACEP Category 1 Credit.

Application has been made with the American Academy of Family Physicians. Determination of credit is pending.

Application has been made to the American College of Obstetrics and Gynecologists for cognates, Formal Learning, for this program.

Nurse Practitioners, Certified Nurse Midwives, Nurses and Physician Assistants
The ANCC contact hours awarded for the completion of this educational activity are 15.

The American College of Nurse Midwives and the American Academy of Physician Assistants accept Category 1 CME approval from organizations accredited by the ACCME.

Provider is also approved by the California Board of Registered Nursing, Provider # CEP2335, for 18 contact hours.

Pharmacology Credit for Prescriptive Authority
A maximum of 5.75 hours in pharmacology credit for prescriptive authority may be earned by attending this educational conference. Sessions awarding pharmacology credit for prescriptive authority are denoted with an Rx.

Each participant should claim only those hours of credit that he/she actually spent in the educational activity.

Certificates of completion/credit will be given to participants at the conclusion of the conference.

CRUISE RATES, DEPOSITS & FINAL PAYMENT REQUIREMENTS
All cruise rates quoted are per person, based on double occupancy, and do not include conference tuition, cruise fees of $159 per person, taxes of $46.54 per person (subject to change), a fuel surcharge of $5 per person/per day (for 1st and 2nd guests only), airfare, gratuities, ground transportation to/from the cruise ship, nor optional Royal Caribbean CruiseCare Protection Plan. All fares are per person in U.S. dollars.

A deposit of $600 per person (including 3rd and 4th passengers in cabin, if applicable), is due to Symposia Medicus at the time of booking. Final payments are due to Symposia Medicus no later than Friday, June 27, 2008. Payments not received by the due date are subject to cabin cancellation with no refunds.

Royal Caribbean will provide travel documents to Symposia Medicus approximately two weeks prior to sailing. Travel documents are issued only after final payment has been received. Symposia Medicus will forward your travel documents to you after we have received and reviewed them for accuracy. You will receive your final documents approximately 10 days prior to the sailing date.

WHAT IS INCLUDED IN CRUISE FARE?
The price of your cruise includes ship accommodations, around the clock dining options, most beverages, nightly entertainment and most onboard ship activities. The cruise fare does not include taxes, cruise fees, fuel surcharge, airfare, ground transfers, gratuities, nor items of a personal nature such as shore excursions, telephone calls, laundry and dry cleaning, medical, barber and beauty shop services, alcoholic beverages, select non-alcoholic beverages, and meals at specialty restaurants on the ship.

CANCELLATION POLICY

Per Royal Caribbean's policy, a full refund (except amounts paid for CruiseCareš Protection Plan) will be made for written cancellations received by Symposia Medicus no later than March 11, 2008. Passengers, who cancel after that date, for any reason, including medical or family reasons, are subject to the following per person cancellation fees.

Up to March 10: Full Refund
Between March 11 and May 9: $100 per person
Between May 10 and July 8: $200 per person
Between July 9 and August 7: 50% of total paid
After August 7: No Refunds

Name changes require the prior approval of Royal Caribbean International and are subject to additional fees. Cruise tickets are non-transferable. Name changes are considered reservation cancellations and are subject to cancellation fees.

CRUISECARE VACATION PROTECTION PLAN
Royal Caribbean's CruiseCare® Vacation Protection Plan provides reimbursement of non-refundable cancellation penalties up to the total value of your cruise should you cancel your cruise for certain specified reasons. As a special feature, if you voluntarily cancel your cruise for any other reason not covered by the insurance portion of CruiseCare, Royal Caribbean may give you up to 75% of the normally non-refundable cancellation charges in future cruise credits.

Royal Caribbean CruiseCare® rates vary depending on the cost of your cruise, and airfare (if you book your air through Symposia Medicus/ Royal Caribbean). Below is a chart showing the cost per person for CruiseCare Insurance. PLEASE NOTE: Prices are subject to change.

PASSPORT INFORMATION

Passports are required for this cruise. If you don't have a passport, please allow yourself 6-8 weeks to receive it after you complete and submit the application. Your passport cannot expire within 6 months of the voyage termination date.

Guests who do not possess the proper documentation may be prevented from boarding their flight or ship, disembarking or entering a country, and may be subject to fines. No refunds will be given to guests who fail to bring proper documentation. For additional passport requirements and information on obtaining or renewing a passport, visit the State Department's website http://www.travel.state.gov/passport.

When you purchase Air Arrangements through Royal Caribbean (Air/Sea Package), you can expect the following:

Round-trip air transportation to and from the port of embarkation and debarkation.

Ground transportation from your arrival airport to and from the pier and from the pier to your departure airport.

Airport Greeting: You will be met by a Royal Caribbean representative at either the baggage claim area, or outside of customs for international flights, and escorted to your ground transportation vehicle.

Typically, your flights will be assigned by Royal Caribbean 60 days prior to the date of your departing flight. Guests are advised to re-confirm flights and times within 48 hours of departure directly with the assigned air carrier.

Royal Caribbean International's Custom Air Department can handle special requests. Options include scheduling your travel to allow for extended stays or early/late flight schedules, booking guests who wish to be sure they will travel together, booking premium-class air seats on your preferred carrier, competitively priced business-class upgrades (subject to availability), booking preferred airlines/ schedules, or requesting an alternate airport if you do not live near one of the offered gateways. Custom air tickets may not be available fewer than 7 days prior to the sailing date. For custom air requests fulfilled and accepted, there will be a $75 per person non-refundable service charge and additional airfare costs may be applied. Any changes to a custom air request may be subject to additional fees.

You may also reserve and ticket your own air directly through the airlines or a travel agent of your choice. However, if you make your own air arrangements, you must allow a minimum of three hours at the beginning and end of your cruise to allow for transfers, customs clearance, and airport security checks.

GROUND TRANSPORTATION
If you choose to make your own air arrangements, you may buy a ticket to board a Royal Caribbean bus to and from the ship (on the day of boarding or departing the ship). Purchases must be made a minimum of two weeks prior to sailing, and flight information must be provided in order to confirm your transfers. You will be sent additional information and an enrollment form for transfers upon receipt of your registration.

DINING ABOARD SHIP
When you make your cruise reservations, you need to request dinner seating preference times of either 6:30pm or 8:30pm. Your seating time will be noted on your cruise ticket. If preferred dining is not available, you can be waitlisted for your preferred time. Please inform us of other passengers with whom you would like to sit at your table. On your cruise, you can expect two formal nights, and one smart casual night. All other nights are casual. Complimentary room service is also available 24 hours a day.

GRATUITIES ABOARD SHIP
Cabin gratuities of $68.25 per person or suite gratuities of $84 per person must be pre-paid and will be included in your final balance. This covers your stateroom attendant, head waiter, dining room waiter, and assistant waiters. An additional 15% gratuity will be added to your bar bills or wine checks on board the ship.

WHAT KIND OF CLOTHING/ITEMS TO BRING
Sweaters, lightweight jackets, jeans, slacks, and sports shirts are fine for daywear. Comfortable walking shoes are a must. An umbrella, light raincoat, and rain hat are suggested as are cameras, binoculars, and a swimsuit (towels are provided).

For evening wear, count on three basic types of attire:

Casual: Dresses or slacks and blouses for women, sport shirts and trousers for men

Smart Casual: Dresses or pantsuits for women; jackets for men

Formal: Cocktail dresses for women; suits and ties or tuxedos for men

Fisher-Price Playgroups
These interactive playgroup sessions are for babies and toddlers ages 6-months to 36-months:

"Aqua Babies" (6-18 months)

"Aqua Tots" (18-36 months)

These sessions were developed by early childhood experts at Fisher-Price. Each 45-minutes session offers age-appropriate activities that expose children to new experiences while combining playtime with learning time. Parents must accompany their children in playgroups.

ADVENTURE OCEAN FOR KIDS
Adventure Ocean is a complimentary program of fun and educational activities open to all guests ages 3-17. Children are separated by age:

"Aquanauts" (ages 3-5 years) - Note: Children must be 3 years of age, toilet trained, and pull-up diaper free to participate in the program.

"Explorers" (ages 6-8 years)

"Voyagers" (ages 9-11 years)

Young Teens (ages 12-14 years)

Teens (ages 15-17 years) You can feel confident leaving your children knowing that every member of their Adventure Ocean staff holds a college degree in education, recreation, or related field and has experience working with children ages 3-17. All are certified in CPR.

BABYSITTING
Children must be at least 1 year old. Babysitting is offered both in a group setting, and individually in your stateroom. In stateroom babysitting must be booked through the Guest Relations Desk (Purser's Desk) at least 24 hours in advance. It cannot be booked prior to the cruise vacation and there is no guarantee that sitters will be available. The service is offered when personnel is available.

Approximate Rate: $8 per hour for one or two children within the same family or $10 per hour for three children within the same family. Times and prices are subject to change without notice. Payment must be made in cash.
