• Tuition includes continuing education certification, extensive conference syllabus, buffet
breakfast on Wednesday, Thursday, and Saturday (for conference attendees only), group
reception, and refreshments.

• Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card.

• Conference registration will be confirmed in writing and more information on travel, hotel
and group tours will be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition
processing fee will be assessed. No refunds will be made if cancellation is
postmarked after August 13, 2008, however, tuition less 10% processing fee, may be
applied to a future Symposia Medicus conference. In the event of conference
cancellation by Symposia Medicus, full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act.

CONFERENCE LODGING INFORMATION Make your lodging reservations as soon as possible by using the form in this brochure. Our block of rooms will be on a space and rate available basis only after July 17, 2008. Rooms may sell out before July 17, 2008, and room rates may increase. PLEASE RESERVE EARLY!
The Tenaya Lodge reflects the grandeur of Yosemite with its elegance and old-world charm. The lodge has two full-service restaurants, a deli, a lounge, weekly outdoor barbeque, and room service. The lodge also has a complete health spa, indoor and outdoor pools, a whirlpool, and an exercise room. For kids, the organized activities at Camp Tenaya will keep them busy and happy. Golfing, horseback riding, mountain biking, fishing, and park tours via the steam-driven railway are all located nearby.
CONFERENCE GROUP RATE:
Deluxe Rooms: $174.00/night (Single/Double Occupancy)
• Note: All reductions to a guest's length of stay must be received 48 hours or more
prior to arrival. If a reduction in room nights is made within 48 hours of arrival, the
guest will be required to pay for the full original length of stay.
• Rate does not include current 10% tax (subject to change without notice), meals,
nor gratuities.
• Children 17 and under stay free in room with adults, and with existing bedding.
• A $15 per person charge will be added for triple & quad rooms (including rollaways).
• A two nights' deposit must accompany your registration in order to reserve a room. The
balance of lodging payment, as well as any incidental charges, is due to the hotel upon
check-out.
• Check-in time is 3:00pm and check-out time is 11:00am.
• A 30 minute break has been allotted in our conference schedule on the last day for those
of you who need to check out.
• Conference participants and their guests may receive this group rate by using the form in
this brochure, by calling Symposia Medicus for reservations at (800) 327-3161 or
(925) 969-1789 with Visa, MasterCard, or Discover Card for deposit, or by registering
on-line on our website at www.symposiamedicus.org.
• Hotel cancellations must be made in writing and you will receive a full refund if
postmarked by July 16, 2008. Between July 17, 2008 and August 13, 2008, one night's
deposit is forfeited. After August 13, 2008, no hotel refunds can be made. All
cancellations must be made in writing and postmarked by the dates stated above.
AIRLINE TRAVEL INFORMATION The closest airport to Yosemite is Fresno Yosemite International Airport, however, lower fares can often be found by flying into San Francisco International Airport. San Francisco is an approximate three to four hour drive to Yosemite and Fresno is an approximate one and a half to two hour drive to Yosemite.
Symposia Medicus has arranged group discounts with Alaska, American, Northwest, and United Airlines for this
conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through your travel agency or through the airlines' Convention Desk at the numbers listed here.

Symposia Medicus has reserved conference group rate accommodations at the Tenaya Lodge Resort, located approximately 2 miles south of the entrance into Yosemite National Park. Accommodations are available for conference participants and their guests at the rate listed below. If you wish to extend your stay, this rate will be available 3 days prior and 3 days following the conference, based on availability.
