CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition includes continuing education certification, extensive conference syllabus, continental breakfast, group reception, and refreshments.

• Register on-line at www.symposiamedicus.org OR by using the form in this brochure.

• Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card. If paying by check, make payable and send with registration form to Symposia Medicus, 399 Taylor Blvd., Suite 201, Pleasant Hill, CA 94523-2200.

• Telephone registrations will be taken with Visa, MasterCard or Discover Card. Telephone registrations should be made Monday-Friday, 8:30 am-5:00 pm (Pacific Time) by calling (800) 327-3161 or (925) 969-1789. Registrations may also be faxed with credit card number to (925) 969-1795.

• Conference registration will be confirmed in writing and more information on travel, hotel and group tours will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after June 18, 2008, however, tuition less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your own financial authority for more information.

CONFERENCE LODGING INFORMATION
Symposia Medicus has reserved a variety of accommodations at group rates at the beautiful Grouse Mountain Lodge. Lodging is available to registrants and their guests at the rates listed below. These rates will also be available three days before and after the conference (based on availability) if you wish to extend your stay. In order to qualify for our group rates, register on-line at www.symposiamedicus.org, use the reservation form in this brochure, or call us for reservations at (800) 327-3161 or (925) 969-1789.

Our room block will be released on May 23, 2008 and accommodations will be on a space and rate available basis after that date. Rooms may sell out before our release date, so please reserve early.

Grouse Park View: $189/night
Max Occupancy: 4
These spacious rooms offer one king or two queen beds, full bath with double vanity & hairdryer. The view overlooks the Grouse City Park.

Golf - Nordic View: $199/night
Max Occupancy: 4
Identical to Grouse Park View rooms, except view is of the Whitefish Lake Golf Course/Nordic Trail.

Deluxe: $209/night
Max Occupancy: 4
These top floor rooms offer a vaulted ceiling and a view of the Whitefish Lake Golf Course/Nordic Trail.

Corner Deluxe: $219/night
Max Occupancy: 4
These first & 2nd floors corner rooms have large windows on both sides. Choose from one king or two queen beds. Rooms have view of either Grouse City Park or Whitefish Lake Golf Course/Nordic Trail. Sold on a first come, first serve basis. Very limited rooms available, so reserve early.

• Rates are per room, per night, and do not include lodging tax (currently 9%), meals nor gratuities

• Additional guests in room 13 years or older are $10/each per night. Children 12 and under stay free in room with adults with existing bedding

• Maximum occupancy is 4 people and cannot be exceeded

• Two nights' room deposit must accompany the lodging reservation form. The balance of lodging payment, as well as any incidental charges is due to the hotel upon check out

• Check in time is 4:00 pm and check out time is 11:00 am

• Hotel provides complimentary hotel shuttle to/from airport and Amtrak station. Please call the hotel to make reservations.

• Hotel cancellations must be made in writing and a full refund will be made if cancellation is postmarked on or before May 21, 2008. Between May 22, 2008 and June 18, 2008, one night's deposit is forfeited. If cancellation is postmarked after June 18, 2008, no reservation deposit refunds can be made.

AIR TRAVEL INFORMATION
You will fly into the Glacier International Airport which is located in Kalispell, Montana. The travel time from Glacier International Airport to the resort is approximately 15 minutes.

Symposia Medicus has arranged for discounts with Alaska, Northwest, and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the numbers listed here or through your own travel agent.
