CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition:

If postmarked on/before May 21, 2008:
Physicians: $580.00
Nurses, Residents, & PA's: $480.00

If postmarked after May 21, 2008:
Physicians: $630.00
Nurses, Residents, & PA's: $530.00

• Tuition includes continuing education certification, extensive conference syllabus, group reception, breakfast (Wednesday, Thursday and Saturday), and refreshments.

• Register on-line at www.symposiamedicus.org OR by using the form in this brochure.

• Tuition may be paid by check, money order, Visa, MasterCard, or Discover. If paying by check, make payable and send with registration form to Symposia Medicus, 399 Taylor Blvd., Suite 201, Pleasant Hill, CA 94523-2200.

• Telephone registrations will be taken with Visa, MasterCard or Discover. Telephone registrations should be made Monday-Friday, 8:30am-5:00pm (Pacific Time) by calling (800) 327-3161 or (925) 969-1789. Registrations may also be faxed with credit card information to (925) 969-1795.

• Conference registration will be confirmed in writing and more information on travel, hotel, and activities will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after June 25, 2008, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. Please consult your own financial authority for specific interpretation.

CONFERENCE LODGING INFORMATION

Symposia Medicus has reserved a variety of accommodations at group rates at the beautiful Big Sky Resort. Lodging is available to registrants and their guests at the rates listed below. These rates will also be available three days before and after the conference (based on availability) if you wish to extend your stay. In order to qualify for our group rates, you may use the reservation form in this brochure, call us for reservations or register on-line at www.symposiamedicus.org.

Our room block will be released on May 30, 2008, and accommodations will be on a space available basis after that date. Rooms may sell out before our release date, so please reserve early.

Big Sky Resort is a full-service resort with restaurants, heated indoor/outdoor pools, hot tubs, steam room, sauna, and spa.

ROOM RATES:

Huntley Hotel Rooms (Main Lodge - contains conference center & spa)
$152/night (Single/Double Occupancy)
Max. Occupancy: 4; Approx. 375 sq. ft.
Rooms DO NOT have air conditioning. Rooms have two queen size beds, mini refrigerator, TV, one bathroom, wet bar, coffeemaker, iron & ironing board, and blow dryer.

Shoshone 1-Bedroom Suite (Attached to Huntley Lodge - most rooms face Lone Mountain or Andesite Mountain)
$218/night (Single/Double Occupancy)
Max. Occupancy: 4; Approx. 790 sq. ft.
Has a private bedroom with one queen bed, 1-3/4 bathrooms, Murphy bed and double sleeper sofa in living room, fully equipped kitchen, A/C, gas fireplace and a small balcony.

Shoshone Corner 1-Bedroom Suite (Attached to Huntley Lodge - most rooms face Lone Mountain or Andesite Mountain)
$229/night (Single/Double Occupancy)
Max. Occupancy: 4; Approx. 800 sq. ft.
Has a private bedroom with one queen bed, 1-3/4 bathrooms, Murphy bed and double sleeper sofa in living room, fully equipped kitchen, A/C, gas fireplace and a small balcony. Square footage is larger than the Shoshone Suite.

Shoshone Loft Suite (Attached to Huntley Lodge - most rooms face Lone Mountain or Andesite Mountain)
$315/night (Single/Double Occupancy)
Max. Occupancy: 6; Approx. 1140 sq. ft.
Downstairs set up same as Shoshone Suite (no Murphy Bed). Open spiral staircase goes up to loft. Has the same amenities as the suite, plus another queen bed or two double beds, and bathroom in the loft.

Summit Studio (Euro-western luxury hotel in the heart of the Mountain Village. A two-minute walk to the Conference Center)
$197/night (Single/Double Occupancy)
Max. Occupancy: 2; Approx. 480 sq. ft.
Has a queen size pull-down Murphy bed, full kitchen, dining room, A/C and private bath.

Summit Hotel Room (same area as studio)
$206/night (Single/Double Occupancy)
Max. Occupancy: 4; Approx. 440 sq. ft.
Has two queen size beds, A/C, wet bar and private bath.

Summit 1-Bedroom Suite (same area as studio)
$261/night (Single/Double Occupancy)
Max. Occupancy: 4; Approx. 880 sq. ft.
One queen bed, one sleeper sofa, full kitchen, dining area, A/C and two bathrooms.

Summit 2-Bedroom Suite (same area as studio)
$353/night (Single/Double Occupancy)
Max. Occupancy: 6; Approx. 1300 sq. ft.
One queen bed in bedroom, two queen beds in second bedroom, and queen size sleeper sofa in living room; has three bathrooms, full kitchen, dining room, A/C and fireplace.

• Rates are per room, per night, and do not include lodging taxes and resort fees (currently 17%), meals nor gratuities.

• Check in time is 5:00pm and check out time is 10:00am for Shoshone & Summit; noon for Huntley.

• Children 10 and under stay free in room with parents with existing bedding.

• Maximum occupancy in rooms cannot be exceeded.

• More than two people in room, add $10 (except for Summit Studio).

• Two nights' room deposit must accompany the lodging reservation form. The balance of lodging payment, as well as any incidental charges, are due to the hotel upon check out.

• The Big Sky Resort is a smoke-free environment.

• Conference participants and their guests can receive these rates by either using the form in this brochure, by calling Symposia Medicus for reservations at (800) 327-3161 or (925) 969-1789 with Visa, MasterCard or Discover Card number for deposit, or register on our website at www.symposiamedicus.org.

• Reservation cancellations must be made in writing and a full refund will be made if cancellation is postmarked on or before May 28, 2008. Between May 29, 2008, and June 25, 2008, one night's deposit is forfeited. If cancellation is postmarked after June 25, 2008, no reservation deposit refunds can be made.

AIRLINE TRAVEL INFORMATION

You will be flying into Bozeman, Montana, which is approximately one hour from the resort.

Symposia Medicus has arranged for discounts with Alaska, Northwest, and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the numbers listed here or through your travel agent.
