ACCREDITATION Symposia Medicus is accredited by the Accreditation Council on Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
Symposia Medicus is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation (ANCC).
CONTINUING EDUCATION CREDIT Application has been made to the American College of Obstetricians and Gynecologists (ACOG) for Cognates, Formal Learning. Determination of credit is pending. Nurse Midwives, Nurse Practitioners,
Nurses and Physician Assistants The contact hours awarded for the completion of this educational activity are 15.5
Provider is also approved by the California Board of Registered Nursing, Provider # CEP2335, for 18.6 contact hours. Pharmacology Credit for Prescriptive Authority Each participant should claim only those hours of credit that he/she actually spent in the educational activity.
Certificates of completion/credit will be given to participants at the conclusion of the conference.
CRUISE RATES, DEPOSITS &
FINAL PAYMENT REQUIREMENTS A deposit of $300 per person (including 3rd and 4th passengers in cabin if applicable), is due
to Symposia Medicus at the time of booking. Final payments are due to Symposia Medicus no later than Friday, May 23, 2008. Payments not received by the due date are subject to cabin cancellation with no refunds.
Royal Caribbean will provide travel documents to Symposia Medicus approximately two weeks prior to sailing. Travel documents are issued only after final payment has been received. Symposia Medicus will forward your travel documents to you after we have received
and reviewed them for accuracy.
WHAT IS INCLUDED IN CRUISE FARES CANCELLATION POLICY Up to June 3 - Full Refund Name changes require the prior approval of Royal Caribbean International and are subject
to additional fees. Cruise tickets are
non-transferable. Name changes and departure date changes are considered reservation cancellations and may be
subject to cancellation fees.

Cruise fare / cruisecare® insurance rates PASSPORT INFORMATION It is the sole responsibility of the guest to identify all travel, inoculation and health requirements, and to obtain and have available when necessary the appropriate valid travel documents. Guests who do not possess the proper documentation may be prevented from boarding their flight or ship, disembarking or entering a country, and may be subject to fines. No refunds will be given to guests who fail to bring proper documentation. For additional passport requirements and information on obtaining or renewing a passport, visit

AIR TRAVEL INFORMATION Samples of round-trip, per person, fares from selected cities include: When you purchase Air Arrangements through Royal Caribbean, you can expect
the following: Round-trip air transportation to and from the port of embarkation and debarkation; Airport Greeting: You will be met by a Royal Caribbean representative at either the baggage claim area, or outside of customs for international flights, and escorted to your ground transportation vehicle; and Ground transportation from your arrival airport to the pier and from the pier to your departure airport.
Generally, air transportation will be arranged for same-day departure. However, guests may be required to travel on red-eye flights or arrive up to two days before sailing. Guests may be required to make connections with different airlines. Your flight may require you to make connections and/or stops en route to your final destination. Connections may be on different airlines. Royal Caribbean reserves the right to select the carrier, flights, routes, and layover city (if any). Typically, your flights will be assigned by Royal Caribbean 60 days prior to the date of your departing flight.
Royal Caribbean International's Custom Air Department can handle special requests like extended stays and upgrades. For custom air requests fulfilled and accepted, there will be a $50 per person nonrefundable service charge and additional airfare costs may be applied. Any changes to a Custom Air request may be subject to additional fees.
You may also reserve and ticket your own air directly through the airlines or your travel agent. However, if you make your own air arrangements, you must allow a minimum of three hours at the beginning and end of your cruise to allow for
transfers, customs clearance, and
