• Conference registration will be confirmed in writing and more information on travel and hotel will be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after May 10, 2008, however, tuition, less 10% processing fee, will be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

HOTEL RESERVATION INFORMATION Symposia Medicus has reserved a block of rooms at our group rates of $99/night
single/double occupancy Sunday - Thursday and $109.00/night single/double
occupancy Friday - Saturday at the Riviera Hotel & Casino. The Riviera is a self-
contained casino and resort with everything to offer its guests. The hotel, located right
on the famous Las Vegas Strip, combines a resort atmosphere with the glamour and excitement of Las Vegas.
Lodging is available to conference participants and their guests at the rates listed below. These rates will also be effective three days before and three days after the
conference if you wish to extend your stay in Las Vegas (based on room and rate availability).
Rooms in our block will be released on April 1, 2008, and will be available after that date on a space and rate available basis. Reserve early as rooms may sell out before release date.

• A 30-minute break has been allotted in our conference schedule on the last day, for those of you who need to check out of the hotel.
• Hotel cancellations must be made directly with the hotel 72 hours prior to arrival or you will be charged a cancellation fee equal to your one night's deposit & tax.
