CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition:

If postmarked on/before May 21, 2008:
Physicians: $580.00
Residents, PA's & Nurses: $480.00

If postmarked after May 21, 2008:
Physicians: $630.00
Residents, PA's & Nurses: $530.00

• Tuition includes continuing education certification, extensive conference syllabus, continental breakfast, group reception, and refreshments.

• Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card. If paying by check, make payable and send with registration form to Symposia Medicus, 399 Taylor Blvd., Suite 201, Pleasant Hill, CA 94523-2200.

• Telephone registrations will be taken with Visa, MasterCard or Discover Card. Telephone registrations should be made Monday-Friday, 8:30 am-5:00 pm (Pacific Time) by calling (800) 327-3161 or (925) 969-1789. Registrations may also be faxed with credit card number to (925) 969-1795.

• Conference registration will be confirmed in writing and more information on travel, hotel and group tours will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after June 25, 2008, however, tuition less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your own financial authority for more information.

CONFERENCE LODGING INFORMATION

Symposia Medicus has reserved accommodations at the Tenaya Lodge at Yosemite. Accommodations are available to conference participants and their guests at the rate listed below. This rate will also be available 3 days prior and 3 days following the conference, based on availability.

You should make your lodging reservations as soon as possible by using the form in this brochure, as rooms may sell out before our release date. Our room block will be released on May 30, 2008, and rooms will be on a space available basis only after that date.

The Tenaya Lodge reflects the grandeur of Yosemite with its elegance and old-world charm. The lodge has two full-service restaurants, a deli, a lounge, weekly outdoor barbecue, and room service. The lodge also has a complete health spa, indoor and outdoor pools, a whirlpool and an exercise room. For kids, the organized activities at Camp Tenaya will keep them busy and happy. Golfing, horseback riding, mountain biking, fishing, and park tours via the steam-driven railway are all located nearby.

• GROUP RATE: Deluxe Rooms: $275.00/night (Single/Double Occupancy)
Deluxe rooms are 350 sq ft - featuring a refrigerator, a coffee maker, Wifi, Cable TV and much more. All rooms have a view of the surrounding forest.

• Rate does not include current 10% tax (subject to change without notice), meals, nor gratuities.

• Children 18 and under stay free in room with adults with existing bedding.

• A $15 per person charge will be added for triple & quad rooms (including rollaways).

• A two nights' deposit must accompany your registration in order to reserve a room. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check-out.

• Check-in time is 3:00pm and check-out time is 11:00am.

• A 30 minute break has been allotted in our conference schedule on the last day for those of you who need to check out.

• Note: All reductions to a guest's length of stay must be received 48 hours or more prior to arrival. If a reduction in room nights is made within 48 hours of arrival, the guest will be required to pay for the full original length of stay.

• Hotel cancellations must be made in writing and you will receive a full refund if postmarked by May 28, 2008. Between May 29, 2008 and June 25, 2008, one night's deposit is forfeited. After June 25, 2008, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION

The closest airport to Yosemite is Fresno Yosemite International Airport, however, lower fares can often be found by flying into San Francisco International Airport. San Francisco is an approximate three to four hour drive to Yosemite, and Fresno is an approximate one and a half to two hour drive to Yosemite.

Symposia Medicus has arranged group discounts with Alaska, American, Northwest, and United Airways for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through your own travel agency or through the airlines' Convention Desk at the numbers listed here.
